First Visit

After scheduling your appointment, when you arrive at our office, you will be asked to text or call our office. As soon as everything is perfectly set up for you, our staff will let you know to enter. You will be happily greeted by an office manager.

If you have sent all your information, along with a picture of your insurance card, with the New Patient Form Patient Screening Form links we sent you, you are ready to go. One of our friendly staff will take you back for your x-rays if needed. Once x-rays are taken, you will be seated and will be seen by Dr. Choi or Dr. Lee. Records of what is seen in your mouth that day will be taken.

Most likely you will be serviced with a cleaning on that day. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Dr. Choi or Lee will let you know what they see in your mouth, and go over in full detail, their diagnosis, options for treatments and recommendations. Our office manager will then provide you with a financial plan that gives you an estimated summary with all insurance coverages and your portion.

We schedule you your next appointment, and that will be your first visit with us!

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
